Referral Submission
At this time, we are only accepting referrals or a patient who is 60 years or older, lives in Kentuckiana and requires transportation to life-sustaining medical treatments from our medical provider referral partners.

Resource Screening
We assess whether the patient may qualify for existing transportation support such as:
Insurance-based benefits
Medicaid or Medicare transportation options
Veterans services
Community or nonprofit transportation programs

Filling the Gap
If transportation resources are unavailable, insufficient, or delayed due to waitlists, Beyond60 Solutions may use program funds to ensure the patient receives timely transportation.
