Why This Matters

Missed medical treatments can lead to serious health complications, hospitalizations, and loss of independence. Transportation should never be the reason someone cannot receive life-sustaining care.

This Transportation Pilot Program is designed to test a compassionate, cost-effective way to reduce missed appointments and improve health outcomes for older adults.
